To become an approved Medicaid provider, you must complete:

State Application & Licensing - $290 (one-time) - Covers state Medicaid application processing, enrollment setup, and credentialing assistance.

Background Checks -Varies by state - All members must successfully pass state and federal background checks (APS & CPS included) before client assignment.

These fees are paid directly to cover state processing costs and compliance services — separate from the $55 Enrollment membership Application Fee.

*The 25 % partnership share supports client case management, billing, and administrative support provided by Almost Family Personal Care LLC.

3️⃣ Get Approved & Assigned - Once approved, we match you with clients seeking services under Medicaid waiver or home-care programs.

4️⃣ Begin Providing Services & Earning - Bill Medicaid for authorized hours. A 25 % agency service fee is applied to each client’s monthly billing.
